RConnection constantly looks at the “big picture” and tries to identify larger pieces which may be missing in local services. These "gaps" in the continuum of care are addressed here in a community-based approach. By creating a network where there is a forum for discussion of both existing and emerging issues, RConnection seeks to identify solutions, create public awareness of available local resources in Rutherford County, and help furnish community feedback to local providers |

What is the Continuum of Care? Why is it important?
As shown below, a “Continuum of Care” comes from the idea that there are many equally important issues occurring at the same time which impact a person’s ability to prosper. No matter how unimportant one component may seem, if it fails, it directly impacts the performance of the whole support system. We have found there are no simple solutions, but with a team effort of the Continuum of Care, many resources are offered in the local area to resolve issues as they arise for families in Rutherford County.
